Le promoteur de parcours inclusif : coordonner la logique de parcours dans le médico-social sur un territoire

Abstract : The care pathway logic instituted by the laws of 2002 and 2005 aims to change the support of people with disabilities in order to promote their empowerment and inclusion (Bauduret, 2017). This necessarily requires cooperation between professionals on a territorial scale. (Loubat, 2017 ; Bloch et Hénaut, 2014). In this article, after regarding the notions of coordination and cooperation, (Moine, 2006 ; Paquot, 2011 ; Bartoli et GOzlan, 2014 ; Jaeger, 2010 ; Dollet, 2014), we propose the definition of a new profession “the promoter of inclusive care pathways “resulting from an immersion in the sector over more than a year and a half. This research highlights the importance of an interface actor in a territory. The promoter of inclusive care pathways will ensure a relevant and adapted relationship with disabled persons, follow and accompany in the coordinated healthcare program and design the accompaniment in complementarity with the partners of the territory and work in a network. At the interface of different structures, at the service of the realization of the life project of the person with a disability, it will be a key actor in the inclusion and continuity of the life course.
